Transition Planning
“Transition” – the passage from one stage or place to another – can be both exciting and, at times, overwhelming for families.
It is important to plan for transition to adult supports and services before a child turns 18.
The FSCD program recognizes that all children/youth and their families go through changes, and that for children with disabilities and their families, the transitions may be more complex or require more proactive planning.
Transitions are likely to be more successful and satisfying for parents if they are well-planned, include multiple opportunities for discussion, involve parents and youth in decision-making, and include adequate preparation for both children and families.
The FSCD program recognizes that there may be a need for transition planning during key changes or events in a child’s/youth’s life:
Examples of key transition times or events may include when:
- a child is first diagnosed or a disability is identified;
- a child returns home from the hospital;
- a child starts an early intervention program;
- a child starts a new school program; or
- a child approaches age 18, marking a transition to adult supports and services
The FSCD program can help families during transitions by providing information and service referral to support the child and family.
