How to apply

Parent or guardians call the FSCD Program in their region to discuss their child’s and family’s needs with an FSCD intake worker.

Based on the conversation with the parent the intake worker will:

  • provide information about the FSCD program;
  • provide information about, and refer the family to, other relevant programs and services if required; and
  • determine if an FSCD worker needs to meet with the family to ascertain if the child is eligible for FSCD program supports and services. If so the intake worker will complete an intake application on the phone, open a file for the child and assign a FSCD worker.

The assigned FSCD worker will contact the parent/guardian to arrange to meet with the child and parent in person. When the worker and parent/guardian meet the worker will:

  • provide additional information about the FSCD program support and services and concern resolution process;
  • review medical documentation or other relevant information from professionals who have worked with the child;
  • discuss the unique needs of the child and family; and
  • discuss what FSCD supports and services and other community programs or supports may help the child and family meet their goals.

All personal information collected by the FSCD program is collected under the authority of the Family Support for Children with Disabilities Act and is managed in compliance with the Freedom of Information and Protection of Privacy Act.

Information a family shares with the FSCD program may be used for the purpose of determining eligibility for the program, processing a family’s application and determining a family’s individual needs.
