Request for Release of Information

For adoptions granted prior to January 1, 2005, identifying information may be released from a sealed adoption record to:

Who?

  • An adopted person who is 18-years-of-age or older.
  • A descendant of a deceased adopted person*.
  • A parent** of an adopted person, once the adoptee is 18 years and six months of age or older.

What information will be released?

  • Identifying information about an adult adoptee or birth parent may be released from the sealed adoption record.
  • No identifying information from the sealed record will be released if a veto has been registered with the Post Adoption Registry prior to the request for information.

* Please provide a death certificate, obituary notice or funeral card.
** Biological parent (who has been named in the adoption record) or an adoptive parent under a previous adoption order, who has been named in the adoption order.
