- Once you have determined that your program or project fits our criteria, download and complete the three forms listed below.
- Remember that you are applying for only one year of funding. Note: when entering the grant amount on the forms, type the numbers only (no decimals) and hit Enter.
- Save the completed documents on your computer in the event that changes may be needed.
